This figure shows an example of the form (page 2) the Minnesota Department of Transportation (Mn/DOT) uses to report monthly incident management performance.

Page 2 of the report provides an incident summary and details of video taped/continuous coverage of the incidents. The incident summary categorizes all incidents in that month by incident type, number of incidents, number of incidents blocking a lane, and total number of vehicles involved. Types of incidents include crash, debris on road, law enforcement, medical, meter activated, other, pedestrian, rollover, spinout, stall, unoccupied stall, and vehicle fire.

The video taped/continuous coverage section provides the video tape ID number, whether the incident was recorded on tape (Y/N), whether the incident was reported on KBEM Traffic Radio (Y/N), the type of incident, the direction, the road on which the incident occurred and cross street, the time, the number of vehicles involved, the time lanes were cleared, and the time in which the incident was completely cleared. Notes for each incident are also provided, e.g., Road closed at 1559 (continued coverage) reopened the left lane at 1604.
